Referrals:
Referrals are done by the patients G.P. Referrals must include:
- Patients Full Name
- Personal Health Number (PHN)
- Date of Birth (DOB)
- Address
- 2 Contact Phone Numbers (if possible)
Urgent referrals must be marked urgent or for direct contact with the physician.
